Wellsource Health Risk Appraisals

Living Well, the university’s employee wellness program administered by the College of Health and Human Performance is offering the Wellsource Health Risk Appraisals for Living Well members and non-members. The non-member appraisal fee is $80. This fee will be waived for the first 100 employees to make an appointment to receive the HRA.

Wellsource is one of the world’s leading providers of health risk appraisals and disease management programs for reducing health risk factors. Appraisals include body composition measurements, blood pressure and cholesterol screenings, and more! Non-member appointments are being scheduled now for April 5 – 16, 2004. Current members of LW may arrange for their appraisal appointment at their convenience.

Please be advised that any information contained in any Health Risk Appraisal at Living Well will be held to the strictest of confidence and will not be distributed or used for any employment or insurance purposes.
