This is a reminder that all employees of the University are subject to reporting and approval requirements regarding their outside activities and potential conflicts of interest.

All faculty and staff must fill out a disclosure form (or forms) and receive approval annually by July 1 (or at the beginning of the fall semester for 9-month faculty) for any activity or financial interest that must be disclosed and approved. Disclosure and approval are also required whenever there are changes in outside activities and financial interests that have been previously approved or prior to engaging in new outside activities or receiving new financial interests that are required to be disclosed and approved. Completed forms must be attached to any reported financial transaction between the individual and the University.

The forms, regulations, and policies concerning disclosure and approval of outside activities and financial interests can be found at: http://generalcounsel.ufl.edu/media/generalcounselufledu/documents/COI.pdf.
College of Medicine faculty and staff must use the college’s online reporting system at: http://coi.med.ufl.edu/.
The Division of Sponsored Research provides a training module (DSR 812), accessible at: http://hr.ufl.edu/learn-grow/job-requirements/compliance-training/.
